01 — UNDERSTANDING THE GLAND
What does the thyroid do?
The thyroid is a small, butterfly-shaped gland at the front of the neck. It produces hormones — chiefly thyroxine (T4) and triiodothyronine (T3) — that regulate metabolism: the rate at which the body uses energy. These hormones influence almost every organ, affecting heart rate, body temperature, weight, digestion, mood and energy levels.
The thyroid's output is controlled by the pituitary gland, which releases thyroid-stimulating hormone (TSH). This feedback loop keeps hormone levels balanced. When the balance is disturbed — the gland producing too much or too little hormone — the effects can be felt throughout the body, which is why thyroid symptoms are so varied.
Thyroid disorders are common, particularly in women, and can develop gradually. Because the symptoms are easily attributed to other causes, they are sometimes overlooked. A simple set of blood tests, interpreted by a specialist, usually gives a clear picture.
02 — THE TWO MAIN PATTERNS
Underactive & overactive thyroid
Most thyroid disorders fall into one of two broad categories, depending on whether the gland is producing too little or too much hormone.
Hypothyroidism
The body's processes slow down. It's the more common of the two, and is often caused by Hashimoto's, an autoimmune condition in which the immune system gradually reduces the gland's function.

Hyperthyroidism
The body's processes speed up. A frequent cause is Graves' disease, an autoimmune condition that overstimulates the gland. Thyroid nodules can also produce excess hormone.

04 — DIAGNOSIS
How thyroid disorders are diagnosed
Thyroid conditions are usually straightforward to identify with the right tests — but interpreting them correctly, and knowing when borderline results need action, is where specialist expertise matters.
Detailed consultation
A full review of your symptoms, medical and family history, medications and any neck changes, alongside an examination of the gland.

Thyroid function blood tests
Measuring TSH alongside T4 (and T3 where needed) to establish whether the thyroid is under- or overactive, and to what degree.

Thyroid antibody tests
Checking for the antibodies that indicate autoimmune conditions such as Hashimoto's or Graves' disease.

Ultrasound & further imaging
Where a nodule or goitre is present, a scan assesses the gland's structure — with a fine-needle sample arranged if needed.

05 — TREATMENT & MANAGEMENT
Restoring balance
Treatment depends entirely on the specific condition and its cause. The shared goal is to return thyroid hormone levels to a healthy range and keep them stable — with careful monitoring over time.

For underactive thyroid
Thyroid hormone replacement restores levels to normal, with the dose tailored to you and reviewed through regular blood tests until stable — and periodically thereafter.

For overactive thyroid
Options include medication to reduce hormone production, and in some cases radioiodine treatment or surgery. Your consultant will explain which approach best suits your situation.

For nodules & structural change
Management ranges from monitoring benign nodules over time to arranging further assessment or onward referral where treatment is indicated.

Who is more likely to be affected
Thyroid disorders can affect anyone, but some people are at higher risk. It's worth seeking assessment if you have persistent symptoms or any of the following.
